ALL Families Need to Upload the following:
IEP or 504 Plan (If applicable)
Address Verification (Approved list below - select two)
PG&E, or Water Bill
Apartment Rental Agreements/ Mortgage Documents
Property Tax Bill/ Current Lease
Notarized Statement from Homeowner
Acknowledging Rental of Living Space and Use of Address
You also need to upload the following:
Emergency Form
Transcript and most recent grades
Copy of Immunization Records and TB Test Results
Copy of Birth Certificate
Copy of Parent's ID (Driver's License/ Passport/ State ID)
Registration Paperwork For All Students/ Families:
Will begin during school day visits and completed at Parent Nights.
Parents who cannot attend the Parent Nights must complete and turn in to STHS Main Office: Course Request Forms on February 16, 2024 and registration packet starting March 1, 2024.
** Please note if Santa Teresa High School is in a DIFFERENT SCHOOL DISTRICT than your current Middle School **NOT all information will "roll over" and families must register with ESUHSD.
