2023/2024 Dance Permission Slips
2023 STHS Student Dance Permission Slip (for current ST students)
All dances are for Santa Teresa High School students. STHS students must comply with the following rules as well as all other school/district rules, and all venue rules in effect. If guests are permitted to accompany a STHS student, they too, must comply with the following guidelines as well as submit a Guest Permission Slip before purchasing a ticket.
For current STHS students, a hard copy of the permission slip, filled out and signed, must be turned in at the dance during the check-in process of the student (and their Guest if applicable) will not be admitted. And no refund will be given.
For guests (if permitted to attend) a hard copy of the permission slip, filled out and signed, must be submitted to the ST School Bank in order to purchase a guest ticket. If applicable, the slip must also include their school’s administrator’s business card attached. The information on the business card must match the information on the permission slip (i.e. the administrator who gave permission).
Hard copies of both permission slips are available outside of Room 803 (ASB), the Front Office, and the Attendance Office.
