ATHLETE REGISTRATION

See the eligibility page or go directly to www.athleticclearance.com to register your student-athlete to tryout for STHS Athletics. 

STEP ONE:

Print BOTH PAGES of the physical form and submit to a licensed physician. 

These are the only forms we are allowed to take per, ESUHSD Board Policy. 

You will not be cleared to tryout/participate in Athletics without this form on file.  

DO NOT START THE CLEARANCE PROCESS WITHOUT THE ESUHSD APPROVED PHYSICAL FORM READY FOR SUBMISSION. THIS SAVES A TON OF TIME! (YOURS AND THE ATHLETIC DIRECTORS)

• Create a profile at Athlete Clearance.. 

• All our athletic forms are paperless

• You can download all forms if you like, once you are logged into Athletic Clearance

• You will need to be cleared to try out AND play sports. 

• Make an appointment with your doctor to get a sports physical ASAP - Don't Wait!

• ONLY START ATHLETIC CLEARANCE IF YOU HAVE A PHYSICAL READY TO SCAN AND SUBMIT. 

STEP TWO: 

Scan all forms in and turn into the online registration website: www.athleticclearance.com before tryouts begin. Coaches are not to collect any STHS Athletic forms or money.

• When uploading a physical, if one file, simply upload to Physical Upload button. 

• If two separate files/scans, upload one file to Physical Upload button, then upload the second file to Additional Forms button. 

• When in doubt, email an Athletic Director.

DURING THE PANDEMIC/DISTANCE LEARNING, EACH STUDENT MUST BE DRIVEN BY A MEMBER OF THEIR HOUSEHOLD, CARPOOLING WITH MEMBERS OUTSIDE YOUR HOUSEHOLD IS NOT ADVISED/ENCOURAGED AT THIS TIME.

FOR DRIVERS OF ANY SCHOOL FUNCTION: Print out form and attach a copy of your Driver's License and a copy of your Proof of Insurance. Turn in to the coach or an Athletic Director, who will then turn it in to the Principal. This form must be on file with the district if anyone uses their own vehicle to transport students. All California driving laws apply. THIS IS FOR ATHLETES & FOR PARENTS/GUARDIANS.

FOR COACHES ONLY::

Injury Accident Report

Fill out the Injury Accident Report Form within 24 hours of any injury or accident that occurs at a practice or contest, regardless of location. This form must be submitted to an Athletic Director at the earliest possible date either in person or scanned and emailed. An Athletic Director will make the appropriate number of copies to distribute to the various entities needing this information.
