On this page you can find information for enrollment, transcript requests, and diplomas. Scroll down to find more information on how to either enroll your student to Santa Teresa High School, or how to receive your diploma and send transcripts to colleges of your choice.

ENROLLMENT STEPS
REQUIRED DOCUMENTATION FOR ENROLLMENT:
Withdrawal papers from the previous school (for current high school students)
Copy of transcript or last report card (for entering freshman, verification of promotion to 9th grade is needed) + IEP or 504 Plan
Birth certificate or Passport
Up-to-date immunization records:
If entering from outside Santa Clara County, records MUST include a TB Mantoux-PPD skin test completed within the last 12 months.
TB tests are good for 4 years, please contact our health tech Mrs. Barbara Rodriguez for more information on Vaccinations. rodriguezba@esuhsd.org or at 408 347-6244
If entering from outside California, records MUST include documentation of the varicella (chickenpox) vaccine or documentation of the disease.
Parent/Guardian picture ID
Proof of residence in the Santa Teresa attendance area: 2 bills with parents' name and Santa Teresa address.
Acceptable bills & official mail communications: PG&E, water bill, home mortgage, notarized rental agreement, legal lease.

DIPLOMAS & VERIFICATION
DIPLOMA PICK UP:
Can be picked up at the registrar's office, if you did not pick it up at time of graduation.
You will need a valid ID to pick up.
Missing books and fines must be cleared before you can receive your diploma

EDUCATION & GRADUATION VERIFICATION:

Please include:
Student's signed release/authorization form
Student's full name
Date of birth
Year of attendance and/or graduation
Please allow up to five business days to process
TRANSCRIPTS WITH PARCHMENT
Transcripts must be ordered through Parchment.
You can come to the the registration office to get assistance with using Parchment to order a transcript if needed. Parchment will request date of birth, graduation year, and all names used in school. You will be able to determine where Parchment sends your transcript when using the application.

Please do not request a transcript unless a college has specifically requested one.
TRANSCRIPT STEPS BY SCHOOL TYPE:
CSU or UC Schools: Students do not need to request a transcript UNLESS it is an application requirement for a special program. CSU and UC do NOT accept transcripts as part of the initial application process.
Common Applications/Send EDU: Students do not need to request a transcript. Your counselor will upload the transcript when they complete your common application.
Out of State & Private Schools: Non CSU, UC, or schools not using Common App, such as private colleges and universities and schools out of state may require a copy of your transcript. Please consult their website to determine if a transcript is necessary.
