Please do not upload documents with your online registration. You must submit a copy of all documents to our front office drop box.

Do you own, rent, or share a home?

Submit documents to registration when registering your student.
Make sure you have these additional documents included with your residency documents from the previous steps.
Online Registration Application Summary from step 2 (DO NOT upload documents online. Print out the Online Registration Summary/Application and submit)
Official Birth Certificate (Government issued only, not from hospital); foreign birth certificates must be translated
Parent/Guardian’s Current Driver’s License/Identification Card
Proof of Residency/Address with Supporting Documents (as indicated from Step 3)
Grade Printout or Screenshot from Parent Portal (Does NOT need to be an official report card or transcript)
Immunization Record & Record of Tuberculosis (TB) Test Results
For students already residing in Santa Clara County: Current test not required as long as you provide a previous record showing negative TB test results. If you can't provide a record, please submit a TB risk assessment form completed and stamped by your health provider.
For those students coming from outside Santa Clara County: The TB risk assessment form completed and stamped by a health provider is required.
Current IEP or 504 must be submitted at the time of registration (If Applicable)
Complete Income Eligibility Form- click below for more information and application
All incoming 9th grade families are strongly encouraged to complete this form to help us maintain and increase the services we can provide to students, families and staff. Advantages of qualifying using the Income Eligibility Form include AP test discounts, college application waivers, internet discounts for home and grant opportunities for ESUHSD.
Health Forms - submit forms only if necessary
Migrant Education Program Form- this form should be completed by families that are engaged in agricultural or similiar work
CCGI Opt Out Form- ESUHSD has recently partnered with California College Guidance Initiative (CCGI) to provide each student with a free account that will help them track their academic progress and identify college and university opportunities. Our district will provide CCGI with student information which will allow students to store in their accounts their college application documents, such as letters of recommendations, awards, and resumes. CCGI facilitates important data-sharing between between ESUHSD and California higher institutions which students are interested in attending in order to help smooth transition from high school to college. This data is stored securely with all appropriate safeguards provided by federal and state law.
Participation in CCGI is optional and requires your signature to opt-out. Because of the tremendous benefit of this program to college planning, opting out is not recommended. However, If you do not wish your student’s information to be shared with CCGI, please click below for more information.
Bring ALL COPIES of required documents to the Registrar's office at Santa Teresa High School located in the counseling office in room 109.
