REQUIRED DOCUMENTS FOR REGISTRATION

All families need to upload the following when registering: 

• IEP or 504 Plan (If applicable)

• Address Verification (Approved list below - select two)

  • PG&E, or Water Bill

  • Apartment Rental Agreements/ Mortgage Documents

  • Property Tax Bill/ Current Lease

  • Notarized Statement from Homeowner 

Acknowledging Rental of Living Space and Use of Address

You also need to upload the following:

• Emergency Form

• Transcript and most recent grades 

• Copy of Immunization Records and TB Test Results 

• Copy of Birth Certificate

• Copy of Parent's ID (Driver's License/ Passport/ State ID)

Registration Paperwork For All Students/ Families:

• Enrollment Packet

• Will begin during school day visits and completed at Parent Nights. 

• Parents who cannot attend the Parent Nights must complete and turn in to STHS Main Office: Course Request Forms on February 12, 2026 and registration packet starting March 2, 2026.
