Families must register with the ESUHSD (East Side Union High School District) to complete the enrollment process.

Santa Teresa High School is in a different school district than your current middle school.

⚠️ Do not assume that all information from your current school will automatically transfer.

2. Prepare the Following Required Documents
Upload/Copies of the following:

📜 Transcript / most recent grades
💉 Copy of Immunization Records and TB Test Results/ TB RISK ASSESSMENT FORM
🍼 Copy of Birth Certificate (or Passport / Visa)
🆔 Copy of Parent's ID (Driver's License / Passport / or State ID)
📝 IEP or 504 Plan (if applicable)
🏡 Address Verification select two from the approved list below:
- 🔌 PG&E and/or Water Bill
- 🏠 Apartment Rental Agreements or Mortgage Documents
- 💳 Property Tax Bill or Current Lease
- 🖊️ Notarized Statement from Homeowner
